I think Violet was deliberately provoking both John and Sherlock. She wanted to see if she could get a reaction out of John and she wanted to see how Sherlock would react to that reaction. I think it was her own little form of match-making.
I was hoping for an unexpected snog, but I will subsist on your glorious angst a bit longer. I love how the two of them don't even realize how like old marrieds they already are. I can see why it's so easy for Violet to believe the whole charade.
I have a Theory. Violet said what she said about John's leg to ensure that Sherlock would go out with him. To give Sherlock the idea that John might need his help.

They do know the Nativity story, but I think neither of them caught the reference because it would have required them to be thinking in a different context, one that they just weren't thinking at that moment, too distracted with other things.
