Made with Photoshop 7.0, but it is translatable.

I used this picture, which I found at Getty Images

Step 1:
Choose your picture, crop, resize and sharpen once.

Step 2:
Duplicate your base, drag it to the top and set it to Hard Light 100%

Step 3:
Go to Layer >> New Adjustment Layer >> Color Balance and use the following settings:
Highlights: -52   +6    +31
And set it to Pin Light 100%

Step 4:
Create a new layer, floodfill it with #FF76CD and set it to Overlay 53%

Step 5:
Create another new layer, floodfill it with #E2FEA5 and set it to Soft Light 100%

Step 6:
Go to Layer >> New Adjustment Layer >> Hue/Saturation and use the following settings:
Saturation: +38
Lightness: -3

Step 7:
Duplicate your base, drag it to the top, sharpen once and set it to Soft Light 100%

Step 8:
Create a new layer, floodfill it with #023B6B and set it to Exclusion 52%

And you are done!
It depends on your picture how your icon looks, just play around with the settings ;-)
