Open a new document thats 500 x 300
Open this texture and place over document
Open this texture, set to soft light and place over the first texture
Next, open this texture, set to overlay and place over the first two textures
Now, take your picture and make it transparent by erasing the background. I used this picture. Place over textures.
Then, open this texture, set to soft light and place over the picture and textures
Open this texture, set to lighten and the opacity to 10%
Finally, add your text. I used Century Gothic, 16 pt. I added outer glow

Your done! This should be your result!
