1) Exactly how long has Amy had a Ganger? Either since the white house bathroom incident or the three month gap in Day of the Moon but definitely before The Silence took her in the orphanage.
1a) How long has the Doctor known that this was Amy's Ganger, not her? Well, he knew something was up when the scanner wouldn't give him a straight answer. The sonic could pick out differences in her molecular structure. Then, you've got the Siren in CotBS. The Doctor mentioned something about protein circuitry that allowed the Siren to mimic human form. Also, the way the patients were kept halfway between here and there. That was probably his first real clue and that was probably why we saw the body scan again in that episode.
2) Do we think the Doctor's Ganger survived the explosion? Maybe but I kinda hope not... It bothered me that he could be duplicated so easily.
3) Do we know anything about The Flesh before Series 6? Prisoner Zero had kinda the same properties, didn't he? Also, the eyes looked kinda like Atraxi eyes. Maybe Prisoner Zero was "born" the same way that Cloverfield!Jennifer was?
3a) Is there a possible connection to "Flesh and Stone" from Series 5? It wouldn't be first time an arc word was thrown in prematurely. However, we still have no connection between SitL/FotD and The Silence so there is probably no connection between ToA/FaS and RF/AP. Also, both SitL and FaS are both a play of words.
4) How are we going to survive 3 months before the rest of the episodes? There IS the new Torchwood but that is only out of morbid curiosity because I think the whole show is rubbish. I'll probably be studying over the summer to get back into school mode. I've been out of school for a year and my head's full of junk.
