Hypothetical, obsessive Savers question

Say you were writing a fic involving Relena and Touma. How would you have Relena directly address Touma?

In-show, naturally, she speaks Japanese. It's a Japanese show, she's in Japan, and she's talking to Japanese people semi-often. She's a well-bred young lady, she'll speak so that she's not leaving people out of the conversation, and Franz would have had to speak some Japanese to Touma's mother (I hope, anyway...).

Except she's not Japanese. =P So, do you think her addressing Touma as 'Oniisama' is for the sake of the Japanese audience, or do you think that she actally addresses him as such? I, personally, want to attempt to keep the connotations of 'Oniisama,' so I want her to actually say it. Obviously, she loves Touma a great deal and thinks he's the coolest thing ever. I have the mental image of Relena sick in bed, and Touma sitting with her, teaching her his native language (he does refer to a town in Japan as 'my hometown') as a way to pass the time or something. Something to keep her amused while she's bedridden. And she likes it so much that she starts calling him her Oniisama out of respect or affection for his culture or something. I don't know. Does that make any sense to anyone?
