Looking for a VPS provider

So I'm thinking it's about time to start looking for dedicated hosting somewhere -- with my unstable living situation, keeping the server at home probably isn't such a good idea anymore.

Anyone have any suggestions for good hosting providers?

Absolute requirements:
  • Virtual Private Server
  • Linux (duh)
  • Full root access, including the ability to install/uninstall whatever software I wish
  • 20+ GB disk space
  • A reasonable amount of RAM, bandwidth, etc.
  • Unrestricted use (short of anything illegal). I know some places write terms into their contract such as "no IRC" or "no VoIP" or silly things like that. I want to be able to run an IRC server or a VoIP server (such as Teamspeak) if the mood strikes me.
  • A configurable firewall of some sort (iptables is preferred, but not required)
  • Competent tech support :p

Nice-to-haves:
  • VPS running on Xen (Virtuozzo or OpenVZ are acceptable, but Xen is preferred)
  • Gentoo
  • Backups done by the hosting provider
  • DNS hosting (but if I don't get this, I need at least 2 static IPs)
  • Reverse DNS
