use photoshop CS3

original cap >>

STEP 1
I crop the cap, cut the girl (I use mask layer) and do it 100x100 (I like to work with small sizes)
you have something like this

STEP 2
create a background

Create a new layer below the first layer and fill it with blue #024e5e

STEP 3
it's look too dark
duplicate blue layer - screen 100%
duplicate girl layer - screen 100%

STEP 4
add this texture

to the blue layer on screen 46% and my background is done

STEP 5
create a smoke (if you need you can use .png)
create a new layer under my background, take small soft brush (I think opacity 50%) and paint something like smoke and also blur it, layer on soft light 100%

I make it too light, duplicate this layer with opacity 44%

I use soft brush with the white color, opacity something near 30% (looks like this), layer on soft light 100%
