OK, I'm not quite an expert yet, but I'm going to give you my educated opinion, and then we'll just take it from there. There are two parts of this entry that seem to me to be extremely telling of your issue. Here's the first part:
"But I don't know what it is...it's as if there's some huge part of me that the rest of me just screams for...aches for."
In this section, we establish that one part of you is aching for another part of you. But this, of course, is rather vague. If we want to get specific, we need to look here:
"Like when you skin your knee, and your knee hurts for the parts of it that are left behind."
Do you see how the analogy of this section bears on the other section? You skinned your knee (went through an ordeal) and now your knee (your soul) hurts for the parts of it that are left behind (the parts of your heart that are gone now). The analogy reveals that what you are aching for is a part of you that you left behind. So, to find out why you hurt, you have to ask yourself: What part of me did I leave behind? Once you've got that answer, you'll be better prepared to resolve the pain.
