This gives you an example of you use the ten extra fields to give a sort of logical feel to filling things out. (Ideally the identifier forms, the ones that are smaller than the rest, will be automatically filled in when you load a template.) This is a very specialized template, however. Your standard ones will use the forms in a different way and ultimately, will be easily transferable from one to another.

The one used here is from Kurt's CR Page. The code for the original template is included below, as well as the output from the program.
