Today I had to have a ticket review... it would have been perfect except for 1 thing. according to my boss i didn't verify what website a customer was logging into when they logged into myprov.com.
"you need to verify what web site they are going to."
"they said they were unable to log into myprov.com"
"right, but what website is that?"

"how do you know that?"
"they told me...."
"well, in the future you need to find out"
"i didn't this time?"
"no, you didn't specifically ask"
"so, like 'so you're unable to log into myprov.com; is that the website myprov.com?'"
