I think we might have gotten a longer run of Demon!Dean, but the writers probably felt they wanted Dean to be Dean for the 200th episode. Since that is the episode after this one, they had to move fast to get Dean back to normal.
Cas and Sam talked about the blood to cure Dean having to be sanctified, not purified. So it's possible that because Dean became a demon without breaking in Hell, the cure needed to the blood to be more pure than any human could manage. Plus since they were going with Dean telling Sam that Sam was more of a monster than Dean had been AND they were showing flashbacks, the probably didn't think they could have Sam able to purify his blood enough to cure Dean. At the very least, Sam probably felt like he can't be made pure again after what he did to save Dean. I would also say that considering the number of blood bags Sam too, he may have been afraid that if he used his blood there wouldn't be enough to cure Dean. It would have been nice to see Sam use his blood, but I think there are reasons for him not to. However, it would have been nice if they has stated those reasons.
