I think he would've seen it coming, known it would happen (How could he not? He's not a Time Lord for nothing.), had he stopped to think about it. But he didn't! EXACTLY! That's the thing about Crazy!Doctor - he's so manic he's no longer thinking like a time lord. Of course he has the ability to see it, but he's not allowing himself to, instead he's being completely selfish and going for what he really wants at all costs, and that's Rose. *melts* Oh, he needs her so. *sniffle* It takes Present!Doctor, who is still following the rules and thinking like a time lord to figure it all out - it's a beautiful contrast really.

But it was still completely heartbreaking to see him so scared and fading away and just wanting to be with Rose and telling her he loves her and---*chokes up* I know exactly what you mean... *swipes at own tears* He's so desperate for her that he'll do anything, and then to lose her in such a way... *guh*... and yet, he also gave himself an amazing gift by making the choice to change things - because he made his present self better by arming him with Donna and Rose... oh the wonderful Timey Whimeyness of it. It make my brain swirl and I love it.
