John Watson was not prepared for this.  He knew it would be bad when the explosion caused the door to heat up so intensely that John had use a fire extinguisher on it just so he could open the damned thing.  He knew it would be bad when the smoke immediately stung his eyes so badly that he had to crawl through the rubble to get to his best friend.  He knew it would be bad when Mycroft just stood at the door, leaning on the jamb, watching him play hero without a look of dismay or terror or anything really on his face.

He just didn't think it would be this bad.

Two days later and Sherlock opened his eyes.  Everything hurt, and not in the satisfying way it ought to have done from a case well solved and a Chinese dinner shared with John after that sat too heavy in his stomach.  In fact, the second after he opened his eyes, he vomited onto the floor.  John came rushing to his aid, shoving him back into bed, but not before he saw something he really wasn't prepared for in a cot against the wall.

"John--"  Fear--  Oh God, he was afraid, but how else was he meant to react to see himself across the room?
