Back in the early 1900s and before, hospitals or care institutions existed. Think of the hospitals and "retreats" that existed for the upper class, and they weren't the only ones. And often these were found by religious people and constructed by them as well, not "intended" as a religious place but RUN like one. Often not saying it was religious was a way to hide. They taught you and forced on you all the normal ways religious people abuse or hurt you, forced on you how to live when possible for them. For the sake of their piety and riotousness. Black and native American and other brown people were forced into them whether they had medical disabilities or not, and it was a way to hold you there and make the world a "better place" because death or worship was seen as the cure even as it to them made your life better. They could force non-white people into it, either we had no rights to stop them or they would take a way those rights. WE had to be righteous, not them really. See where this goes? They said you were crazy, forced you to live in pain. These places weren't new, but they were becoming less with the right government enforcement and licensing. It was another form of slavery, sure, but since you were lesser you were meant to live as the LESSER and there was no question or consideration of your rights. They hid the religious part from authorities and told the government they were DEFINITLY only giving them standard medical treatment. These places blamed poor behavior on the patient a lot, or even just an inability to fix themselves - and this was a strong way in which they had to KEEP you there and treat you like the lesser and much less than you deserved.

This is still the case of such modern facilities - hospitals, resident and health facilities - the nuns aren't in charge, unless they are of the self-appointed variety. At least, sometimes you hope the nuns aren't involved, but these secular people are just as bad as nuns on their own and convinced of their qualifications to make you a good, not healthy, person. Because being good is seen as not relying on your health - as if it were possible to ignore your health! And notice my use of the word nun, because these views are still often Christian derived. They may even force you to take a vow of silence. Silent mouths can't tell the government or the police or family and friends what is happening. You're threatened, very often directly. If they force you into isolation, like they do with the barbaric isolation rooms (no bed or seat either), they believe they can somehow force you to be in isolation and this is another avenue in which they can exploit your pain and very life.

I also strongly think that institutions stop being able to see through their own propaganda and ideas. Stop being able to tell the difference between "medical" care and religious care or observance. This confusion leads to more issues down the road, and the employee doesn't like to be blamed when they are to blame. They don't get their way, not so easily, and they scream out in anger and treat you with the malice they contain, and which they do all too willfully.

So obviously, this was not FIXED. Not completely. Since Roman times, Greek times - as I mentioned the status of women but also just people - since hospitals in the 1600s, to the 1800s, early 1900s. It was often a secret and difficult to find out, but also an open secret. Even primitive English people and governments saw improvement in this when oversight was adopted and increased. Which isn't always government oversight.
