In his head, it's not that.
It's kissing, not sex. He can convince himself that they're okay, that he didn't ruin them. They just kiss sometimes, that's all.
It's not like it was completely unnatural for them. Sam never grew out of that phase where he wanted to be Dean's shadow, not even when he finally got that Winchester mouth and every other thing out of it was how Dean had no brain of his own-and if Sam only knew the amount of time Dean stared John down for Sam's random crap, he'd be singing a different song.
And Dean can't say no to Sam. John won't let himself remember every time Dean was the one who played bad cop and told Sam he couldn't go on the field trip, stay the night at some kid's house, or get some stupid, expensive toy he was gonna hate in a week anyway.
It's not like it's random. He didn't just walk into the motel and find them kissing at the table or anything. Dean was hurt bad, and they weren't sure he was gonna be a hundred percent again. Sam was worried and angry and so frustrated at the lack of news that he couldn't even talk right.
When Dean finally woke up, after they took out the tubing and the doctors ran their tests, Sam just needed some kind of reassurance.
