My mother wants to go out and risk her life gently trim it back this afternoon; I feel like we should try to rehabilitate it weave the new vines in downwards and try to get them to trail a bit.

it seems like the bottom might need to be thinned a bit. The shoots will go straight up to get the sunshine, if I am not mistaken.
Training it is a good idea, too.
