I believe that the one-shot listings were an attempt at humor. They don't want to give any example anywhere near the real threshold, lest the exploiters figure out where the line is too soon and start exploiting just below the line.
"The system isn't designed to punish "aggressive" playstyles, only to detect, and prevent, egregious exploitation. The examples provided are pretty explicit as to what you would have to simultaneously defeat to trigger the throttle."
As far as I can tell, a farm that mainly has fire-using enemies (since fire defenses become available pretty early on, much like there used to be Smashing/Lethal farm using the Crazed (insertnamehere) mobs).
It goes a bit beyond that, but that's the gist. It's a series of AE maps with pure fire enemies, perfect for Fire Armor brutes. What makes them egregious is that this was also when people started really playing with the spawn mechanics and created Ambush maps. One tiny room, four or five static spawns, and two destructible glowies. Blowing up a glowie spawns three full ambushes. So in one tiny room there's the equivalent of 11 full spawns of dudes. And the exit glowie as well.
When you add in SSK and the SRSLY difficulty sliders.... An ambush farm at +4/x8 gives you 1-50 in about 5 hours, if you're really cranking. That's really too fast.
