Bloody Mary: the spirit of Mary Worthington, a 19-year-old beauty queen murdered by a plastic surgeon she was having an affair with; he cut out her eyes. In her dying moments, she tried to write his name on the mirror in front of her in her own blood, leaving three letters and a handprint. Bound to the mirror, she hunts down anyone with a secret about something they've done that caused a death. Afterwards, her handprint, and the name of the victim of her victim are left on the back of the mirror, visible under blacklight. Her kills are all the same: first she appears in the mirror with her prey, then becomes their mirror image, accusing them of the crime they've kept secret. Their eyes bleed, and eventually liquify, killing them. To kill her, they first summon her to her mirror - the one she died in front of. Once she escapes, they present her with her own reflection, and this appears to destroy her.
This is very much tied to the actual urban legend/myth. One of the names often mentioned is "Mary Worthington". There are a ludicrous number of variations on this story, perhaps indicating its power over teenage girls. Variations include the use of candles, her name, where you should stand, what her background is, and how many times you have to say her name. Even what she'll do - from striking you dead on the spot, to driving you mad. Or, as in this version, "scratching out your eyes".

Some minor demon activity this week: Sam is having nightmares about Jessica in which, trapped and engulfed in flames on their ceiling, she asks "Why, Sam?" It's also revealed that Sam had nightmares about her death for weeks (3?) before she was actually killed, and said nothing.
At the end of the episode, Jessica is standing, dressed in white, on a corner as the boys pass. Only Sam sees her, and she disappears as they pass.
