First TV show I had self-insertion fantasies about:
If I'm remembering correctly, the Spider-man segments of The Electric Company. Although based on watching Frog, I almost certainly had them before then. Those are just the first I remember.
First fandom in which I interacted (online and in person) with other fans:
Sort of depends on your definitions (yes, yes, I know: hi, rhetorician). I was talking about Star Trek on Tri-State Online (a Cincinnati based BBS) back in '92. I was even in an RPG set on DS9 on that system. I hung out on usenet, mostly as a lurker, for Trek and B5. However, for fandom as I usually think of it, which is including activities like fanfic, vids, etc, I would say The Sentinel.
Pairing in the first slash fanfiction I read:
You know, I'm not actually sure? I think it might have been Garak/Bashir, but I sort of discovered slash in a kind of rush of OMGPEOPLEWRITETHISYAY! rush. I know it was in Trek, I know it was either DS9 or Voyager, but I couldn't tell you the exact pairing. I also know I moved on to Sentinel really, really quickly.
First RPS/RPF I read:
Still remains my one and only bulletproof squick. I think I have read some stuff that had real people, mostly historical real people, as cameos, but as main characters, I just can't. I hasten to add, this is a personal thing, but it's a really, really strong one.
First fanfiction I read that made me think, YES, this is exactly the kind of fanfiction I'd like to write...
Well, my very first story was uncomfortably close to a blatant ripoff of a story by
merryish, if that tells you anything ;).

Okay, first, let's distinguish between the first story I wrote from the first story I published, because like a lot of fans, I was writing fanfic before I knew what fanfic was ;). So there's my teenage self-inserts, there's stuff with OCs that weren't actually self-inserts but were otherwise probably Mary Sues, etc.
The first story I published was gen. The first relationship story I published was Jim/Blair.
First OTP:
I don't really have OTPs. I'm more an OTC person. There's usually one character I fixate on that I'll pair with lots of people. Sometimes, that's my barrier to going from "I like this show/source" to "OMGFANDOM": finding that character. No matter how slashy the pairing is, if one of the characters doesn't grab me by the throat, it's not happening. Also, sometimes finding that character will make me fannish even if I have no one to pair them with.
First RPS/F OTP:

First fannish friend I met in person:
I...don't entirely remember. Kareila, maybe? I met her while Sentinel was still on the air, and I don't remember meeting anyone else before then.
First character I formally roleplayed:
Don't know that I ever did. I set a Mage game in Cascade, but I don't think Jim and Blair ever made an appearance.
