Step 1
Take your picture (
I used this one) and crop it to your liking.
sharpen once
Step 2
Add a Brightness/ Contrast Layer (Layer > New Adjustment Layer > Brightness/ Contrast)

Step 3
Add a Solid Color Layer (Layer > New Fill Layer > Solid Color)
Color: #337097
100% Soft Light
Step 4
Duplicate your base, drag it to the top and set it to 100% Luminosity.
Step 5
Add a Selective Color Layer (Layer > New Adjustment Layer > Selective Color)
Reds: -100 / +22 / +18 / +2
Yellows: +100 / -34 / -40 / 0
Cyans: +100 / -100 / -100/ +100
Magentas: -76 / +30 / -15 / -43
Whites: +100 / +100 / -100 / -47
Neutrals: +12 / +10 / +18 / -10
Blacks: 0 / +12 / +16 / -3
Step 6
Add a Color Balance Layer (Layer > New Adjustment Layer > Color Balance)
Midtones: +22 / -18 / -14
Shadows: -25 / -17 / -14
Highlights: -26 / +7 / -1

Add a Hue/Saturation Layer (Layer > New Adjustment Layer > Hue/Saturation)
Master: 0 / +13 / 0

Step 8
Add a Solid Color Layer

30% Multiply
