Well-conditioned Conditional Errors

These are two grammatical errors that really annoy me. A long time ago I though they were more prevalent in the Midwest than elsewhere but lately I've been seeing these in quotations in the NY Times and in online forums by people from across the country.

1. [conditional + past tense] instead of [conditional + past participle]. I guess "would have went" somehow sounds correct to many people.

2. "if....would have....would have" instead of "if....had...would have". Most people say "I woulda done X if you woulda done Y" which is, like, so wrong.
