Yes, we already knew all of this and it did come up earlier - it was the whole point of the episode 'First Born'. Cain was a 'retired demon' (if there is such a thing). He sold his soul to Lucifer and became a demon in exchange for his brother's soul going to heaven, and the Mark of Cain was what Lucifer gave him when he turned into a demon. Then he trained the Knights of Hell, before turning on them and killing them all (except Abaddon).
Also, Crowley did recognise Cain the first time he saw him (he grabbed Dean and told them they had to get out of there because it was Cain, who was "the deadliest demon to ever walk the Earth") and it was revealed in that episode that Crowley knew exactly what was going on the whole time... he was faking his fear of Cain in order to manipulate Dean into getting the Mark. Cas didn't say anything, I'm guessing, because there was no need to - Dean already knew, and the audience knew.
