For Photoshop. Sorry PSP users.
Includes Selective coloring, Curves, and Levels. I also have .PSD because I think this tut is a little more difficult.

1. Crop picture how ever you want. Self explainitory, but you can always make your cropping really pretty. :D
2. Add a new adjustment layer > curves. RGB: Input: 111, Output: 155. 
Now you should have this: 


3. Add a new adjustment layer > levels. Output Levels: 0 (first box), 225 (second box).
Now you should have this:


4. Add a new adjustment layer > selective color layer.
REDS: 0 0 100 0
YELLOWS: 100 0 100 0
WHITES: 0 -30 -54 0
NEUTRALS: -10 0 20 0

And select ok, and you should have this:


5. Add another new adjustment layer > selective color layer.
REDS: 40 0 0 0
NEUTRALS: 15 0 10 0
BLACKS: 0 0 0 20

And now you should have this:


6. Now you can go back and screen the picture layer twice. Second screen layer, change the opacity to 70%. (For this icon, I did it at the end, but you can always start with screening your icon too.) And then I touched it up a little and blurred any of the rough areas on her face.

And this should be your final result:


TA DA! I really like the way this icon looks now but if you want you can always add a brightness/contrast layer if you want a little more contrast.
