Headcanon that when Igor tended Drumknott's stab wound in The Truth, obviously he had to take his shirt off and so found out about Drumknott being a trans man.

(Since for Igors sex reassignment has never been an issue- after all, the body is a plaything to be done with what you like, so the first Igor to say "I think I'd rather be an Igorina" was met as nothing more than an interesting surgical puzzle and once the clan had sorted out the process, then sex reassignment became all in a day's work, and though it's one of the less widely known services they offer, there are a number of trans men and women who've had their dysphoria sorted out by the Igors on the downlow. When Igorina in Monstrous Regiment talked about gender roles, she meant gender roles: you can declare your own gender, but once you've declared it you're generally expected to follow tradition and behave in kind.)

Once Igor had gotten Drumknott sorted out in the stab wound department, he offered to sort out Drumknott's other problems as well; Drumknott had his breasts removed immediately but was hesitant about the rest- having some excess flesh lobbed off is one thing, but having your organs and bits rearranged requires a bit more thought and mental preparation and isn't something you decide on a whim. He went back later on, but at a time when it was allowable for him to take a few weeks off so he could get away from work and the city and just process.

(Vetinari noticed the lack of breasts immediately, because he's Vetinari, and was very supportive of anything that made Drumknott happier. He also helped him talk himself into bottom surgery and made sure to take care of all of his arrangements so that he could take the time off- in fact he was the one who suggested the time off if Drumknott needed it.)

(One of the good points of Raising Steam is Vetinari's indulgence of Drumknott's train obsession, which gives off the impression that he's actually fond of Drumknott and wants him to be happy for more reasons than just how useful and loyal that makes him.)
