I think Russell sees it as essential that every companion and the Doctor have a bit of a kiss of some description. It’s become a law of modern Doctor Who, ‘cause people love a bit of that, don’t they? They love to speculate on what it is… And of course, as usual, it’s not really a kiss, it’s something else. But you know it’s good, isn’t it - it’s fun… shove these things in, sprinkle ‘em in, keep you guessing.
I admit, I find it hard to believe that an actor as technically accomplished as DT would let his face say anything that he didn't want it to. I can't help but think he was in on RTD's original intention to have Ten fall "wham! both hearts" for the S4 companion and decided to keep a little bit of that in there, OR he just decided to play it that way - perhaps because of the way that Ten treated Martha and he felt it was a kinda natural character progression. But as you say, we'll never know. Whatever the case, CT and Donna were completely oblivious..
And as Maia says, she also seems completely oblivious to how great a CT fanboy David is.
But I maintain Ten was utterly smitten and that I never saw him look that way at ANYONE else - not those cute blondes or the gorgeous Martha. There's a sexual element to the looks he gives to Donna in those caps that doesn't appear elsewhere.
And I'll pitch in another one - not sexual this time. Just adoration :-)

Sometimes I get a little fed up with myself and Ten/Donna and wonder why I care about something so inconsequential and unimportant. Then I see pics like this and remember all over again.
I wanted to add when I read about this in The Writer's Tale I was very surprised, what with all the Rose-love going around.

First of all, I'm not sure why RTD was intent on giving Ten a romance in the first place, starting with Rose.
Had he in fact fallen "both hearts" in love with the Season 4 companion, what do you think the fans would've done? I think fandom would not be happy if he fell in love again (haven't we already been on this ride?) - especially with someone not!Rose and would've completely wigged out if it had been Donna (perish the thought) or "Penny" for that matter (wasn't that the Season 4 companion's name to begin with?).
I have to admit, I mostly skimmed The Writer's Tale. I found it a bit tedious having to wade through so much not-DW-stuff.
