There have been so many, but I think the best was Jenny's death. It was the first death beyond Buffy's S1 death and resurrection of somebody we had actually come to know and care about and the fact that Angelus did it in the fashion he did just added to it. It invoked all kinds of reactions from the characters (Buffy's guilt, Xander lashing out at Buffy and blaming her because she hadn't killed Angel when he first turned evil, the first real display of Giles' anger in going after the Fang Gang of Spike/Dru/Angelus in the aftermath). Joss made it clear that this was permanent and sent a very big signal that, with the possible exception of Buffy, nobody was really safe in his universes. That there are consequences to living in his worlds and more often than not their fatal consequences.
Come to think of it, that was probably the beginning of the "joss kills puppies" meme as well.
The revealed truth about Boyd in Dollhouse, definitely. I think it shocked everyone because we all believed him to be one of the good guys, his fatherly tendencies towards Echo, etc. It was a nice twist because he trusted him and believed him, and then it was like "whaaatt?!" I think at that point it was like now who should we trust?
I also have to say Wash's death in Serenity was definitely shocking, as well. ;__;
Joyce's death. Even now, years and countless rewatches later, I always forget that Buffy finds her at the end of I Was Made to Love You. Shocks me every time.
Wash's death was also very startling. I never, never saw it coming.
