Tier 2 visa extension?

Hello fellow expats, I hope you can help me with my query. I am currently a US citizen in the UK on a Tier 2 general visa. My employer has asked if I can extend my contract for 4-5 months past my origional visa end date. I'd like to take her up on this offer but cannot find correct guidance on this issue on the UKBA site. The only information I could find is within the Tier 2 Policy Guidance (Version 12/2012), which states that in this case I need to make an extension application and the rules for when an extension would qualify (I meet all those stated). Well I am happy to do this but I cannot find an application form, instructions to apply, fees information, or anything that would be necessary (I assume) to actually make the application. I am running on a really tight timeline for this as my visa expires at the end of July and I have travel (within the EU) in June (holiday) and July (an international work conference), so would prefer to apply ASAP if I need to send my passport in (I assume I would need to do this). Any advice on where to get these forms, or from anyone who has been through this process and has a clue about how I can go about it, or any other such help, would be fantastic.
