Okay. Data entry. There are two tools I know about that might work for your purposes.
For both of these, you have the option of lots of different kinds of questions, like choosing from a list, free text entry, multiple choice, etc.
1. Google Forms. My understanding is that it automatically enters stuff from the forms into a spreadsheet. You can also embed it on a webpage (or, presumably, in LJ). Extra bonus: it's free.
2. Survey Monkey. You can create a survey, which can act like a form to collect data, and post the link or send it to whoever you like. They also have really great tutorials and tech support (not that Google doesn't). It seems like they have more results analysis options that Google forms (for example, filtering and cross-tabbing responses). However, I think to download the results as a spreadsheet, you need the paid account. You can buy by the month, but it's $25, so that's not free.
I know just a bit about both of them, but I'd be happy to try to help create some sort of form if you're interested in going down that road.
I also know emmademarais uses some kind of automatic web form for sign-ups to run ficfinishing, but I'm not sure what... Technology!

I hope I didn't turn you off Misfits with my caveat. It really is the most wonderful show I have discovered since Lost in 2008 (well, Downton Abbey is pretty good, too, but its charms are very different). Really edgy and funny and real and awesome.
Uh oh. I've maybe watched four episodes of Misfits already. Crap. You're so right about the realness and edginess of it. It's kind of the opposite of the high-falutin' monologues about destiny on Heroes!
