Character's Name: Soul Eater Evans
Character's Age: teenager. His actual age is unknown. I prefer to make him 15 because of how he appears but rumoured age is 13.
What form will your character's NV take? It'll take the form of a normal cell phone. While mirrors are commonly used to contact Shinigami-sama in the Soul Eater world, their technology is advanced enough to suggest that cell phones exist.
Abilities
Character's Canon Abilities:
As a Weapon, Soul has the ability to transform into a human-sized scythe, and even partially transform his arm from the shoulder down into a blade. Despite the ability to transform into a scythe on his own and perform partial transformation, Soul is greatly hindered by the absence of his Meister Maka Albarn. Soul can also take advantage of Maka's Soul Perception through their Resonance, able to sense other people's Souls as well as she can, as well as her Anti-Demon Wavelength. Like all Weapons, though he's usually seen in the form of a scythe he has the ability to change his shape - called Form Manipulation - but the extent of this change has not been shown yet apart from manipulating the length of his blade.
Black Blood, however, is an ability that he gained after being infected by Crona. Black Blood allows him - through a room in his mind inhabited by a little demon the Black Blood implanted - to invade the blood stream of those he performs Soul Resonance with. Black Blood usually comes with an intense insanity, but Soul is able to overcome that insanity and drive the madness away. With this, Soul also has the ability to create Resonance links with anyone despite the difference in their wavelengths.
At the current point in the manga, Soul is a Death Scythe, having achieved the goal of obtaining 99 corrupt human souls (Kishin) and one witch's soul. This means he's (one of) Shinigami-sama's personal weapon(s), though he's still partnered with Maka.

Soul has no weapons on him. Absolutely none. Cool looking blade arm though.

Point in Canon: Whilst going into the Book of Eibon to retrieve Death the Kid. Soul and the other Spartoi members were put on the rescue mission after Kid saved Liz and Patti from being pulled in themselves. With the help of two of the more friendly Witches - Lisa and Arisa - and the reluctant help of the captured Eruka Frog, the members of Spartoi were able to traverse into the Book of Eibon. Soul will be taken just from before the Chapter of Lust, meaning they've only just entered the book.
Character Personality:
On a first meeting, Soul seems laid back, almost apathetic and aloof. He's constantly in a slouch with a drooping look on his face, hands in his pockets. This is his definition of the 'cool guy' he's trying to be. In many ways, he's the complete opposite of Maka; a total slacker when it comes to schoolwork but more than eager to get into a fight with someone who's been devouring souls for their own greed, especially if it means a soul for himself. Yum. This separates him from other Weapons - most Weapons don't like the taste of souls, yet Soul drools over the idea of chowing down on one.
Immature, headstrong and stubborn seem to be the right words to describe Soul, however one of the things he's gained from years of trying to be 'cool' is that he's positively the most loyal person you could ever meet. He considers breaking promises uncool, and would do anything to protect his friends and Meister. Despite appearing to be apathetic, Soul is capable of being determined and gets people back on their feet in desperate times with the right words.
One of the things he keeps to himself is that he's quite vulnerable to low self esteem, first with his brother Wes and then with other Weapons like Justin. He's reluctant to share these problems with others, coming off as cold when inquired about it, and shares Crona's fear of bonding with people.
After Asura is revived, he becomes aware that he needs to get stronger in order to fight off the madness that was released into the world upon Asura's rebirth, and his headstrong personality is replaced by a more level-headed one. He has the ability to keep a cool mind in dire situations, and also possesses deep thought and shows a more understanding sense of judgment when it comes to others.
Despite being a Weapon, he's also human. He has an appreciation for jazz music and Gothic architecture, plays basketball during wind-down times and sometimes has a dark and sarcastic sense of humor.
Character Plans:
Possibly continue his schooling, and get a part-time job. If any others from his world show up those plans might change according to their choices.
Appearance/PB:
Soul appears to be a teenager, with white hair swept to one side and red eyes. He constantly has a droopy expression on his face and a slouched stance, hands either in the pockets of his pants or jacket. He prefers to wear street-type clothes but has also been shown to look quite snazzy in a suit.

First Person Sample
[When the NV flickers on, it seems to be focusing on the user's eye before they realize they have it zoomed in too much. The image backs away a little to reveal a sullen-looking teenager leaning back against a wall.]
So I'm stuck in another world, huh? [His lip curls into a sneer, revealing sharp shark-like teeth as he rolls his eyes, shaking his head.] Not cool. For once I'd prefer to be stuck inside a book.
[After rubbing his shock of white hair, he turns his attention back to the NV.] I don't suppose anyone's seen any other kids wearing this, have they? [He points to the white and blue uniform he's wearing, reminiscent of a typical Japanese high school uniform.] I'm looking for the one with pigtails and no tits. 'Bout my height. It's kinda important that I find her.
Third Person Sample
Soul let out a small groan, rolling over from where he'd apparently landed on his back, feeling the grass shift beneath him as he did so, pushing himself up onto his hands. It was tentative at first - he felt like throwing up, his stomach churning wildly as another wave of nausea hit him. He gave an 'urp' sound, hand rising to his mouth reflectively. After a few more minutes it lowered and he stood properly, brushing himself down as he glanced around, red eyes searching through the darkness.
"Maka?" No answer. Soul gritted his teeth at the thought of being separated from his Meister, his eyes beginning to adjust to the dim lighting. This was some kind of baseball field. He turned his head, trying to pick up something else in the dark of the night to indicate he hadn't turned up here alone. "Tcht. Did that witch screw us over?" he muttered to himself, slipping his hands into the pockets of his jacket. This certainly looked nothing like how he'd imagined the inside of a book to be like. Soul scuffed his sneaker against the ground, looking up at the moonlight poking through the clouds.
Shadows shifting in the dark caught his attention and red eyes flickered to his surroundings. Placing a hand in his pocket and keeping an eye on the monsters strolling out from the darkness, Soul readied himself. Maka might not be here, but he could damn well keep himself alive until he found her, especially if they had even a chance of rescuing Kid. "I'll kick that guy's ass later for putting me in this situation," he murmured, other arm turning into the blade of a scythe. "This is so not cool."
