This one's easy.
Alien anatomy playing a prominent role in sex. Yes, I know the Doctor's an alien. Yes, I know there are things about him that are different (two hearts, cooler than normal body temperature). Blah, blah, blah. For me, nothing kills a steamy fic faster than a belaboured description of the Doctor's prehensile penis. Ew.
