SIGNED POSTCARD OFFER + CONTEST ENTRY
I only received my copy of "BIRTH" two days ago, and with all the stress of moving and planning trips, I haven't gotten to translate this until now!
If anyone was planning on entering to get the signed postcard or the major-super-awesome contest, then please note:

Here's the gist of what the little insert ticket says (sorry it's kind of sloppy x_x;):
Application Privilege 1
Purchasers of all 3 types will receive a postcard signed by all the members of D!

Purchasers of only 1 type will receive an entry in a drawing to win wonderful privileges!
Course A: Invitation to the dressing room at a concert - 10 people
Course B: D original goods - 100 people
Course C: Original cell phone screen saver - 500 people

To enter, please include the following required items along with application ticket.
- Shipping destination address, full name, daytime contact phone number
- Desired privilege (1 or 2)
- In the case of privilege 2, the application course (A~C)
- In the case of privilege 2 Course C, the email address to which the privilege should be sent

1. We cannot process applications for both privileges 1 and 2 simultaneously with one application ticket.
2. Concerning course A, the concert venue is undecided. Notice will be given along with the notice of winning.
(The chosen [by the company] venue could end up being anywhere nationwide.)
3. People applying for Course C... (something about domains... sorry I didn't have time to finish translating this part!!)
4. Winners' announcement will be replaced by the sending of the commodities.
5. Our company will accept this information with intent to only use it for the sending of privileges.

- The ticket does not say whether you need a Japanese address and phone number. If you have a friend in Japan, it's probably best to ask if you can use their address! (As for phone number, I'm not sure about that... it's probably best to do the same, because of the time difference.)
