Resumees for programmers are especially hard to create. One reason is that the usual rule of "not longer than one page" tends to knock out the better ways to display the information.
HR should be able to give you a record of your titles; you can probably play it slick and say something like "I just wanted to make sure I have the official titles right" :)
Extremely obvious advice: Step 1 really is just to make lists of every position you remember, and all your relevant skills, and so on, on paper or in a word doc or whatever.
You'll leave some out, but you'll probably trigger your memory as you go. Then you'll have the raw material and you can think about how to organize it, which is of course the challenging part because you need to think about how to present your experiences as part of a whole, directed towards some particular category of goal/job/interest. But the process of organizing it also helps you identify what that goal/job/interest might be.
