It was always like this. He knows how to find Sam, doesn’t matter how far he goes and how hard he tries to cover his footsteps - Dean always knows. And he doesn’t think to wonder how he can do that. He knows and that’s all. And Dean has never stopped to think how weird is to be PHD in Samuel Winchester’s tastes. He always thought of that as something natural, some part of his job, of his older-brother-life/mission.
That’s why it hurts so badly when he hear Sam’s voice, so much anger in it:
“You don’t know me. You never did. And you never will”.
It hurts more, the punches and kicks and everything, because it is Sam. And, as usual, he knows exactly what to do to hurt him as badly as he has hurt him:
“If you walk out that door don’t you ever come back!”
