As usual, you have managed to very clearly put into words a lot of my confused thinking!
A couple of things - on the causality glitch that bothered you - I had actually been happy with that as I was under the impression that Dean was to take the girl while she was being operated upon, though when I think about it, I am not sure exactly why I thought that.
Julian Richings as Death - YES!! He has been on screen for mere minutes in total, but has been absolutely chilling yet wonderfully funny for every second of them. And when you are talking about the quality of acting, Jensen's portayal of his utter fundamental fear of Death coupled with his inability to keep his big mouth shut when faced with a scary 'big bad' has been nothing short of genius.
The one thing that scares me the most about the way the show is going with the whole natural order thing is this - does it become inevitable that the show must therefore end with order restored and that Sam and Dean, being who they are, will decide to restore that order themselves - by dying???
Somehow, I don't think Winchester suicide is quite the way to set things right ... I'm not going to worry about it for a good long time!
I totally agree with you on Jensen's superb rendition of Dean absolutely intimidated by Death but still constitutionally unable to stop snarking in conversation with him. Watching that man is a delight - and I will watch Julian Richings any time he turns up on my set!
