It really doesn't. He's had centuries to collect enemies, it's almost a snowball effect. Even so, his enemies are only a small percentage of the beings in the universe - there's an even greater number who respect or even revere him.

Good thing his regenerations change his face too! The guy is amazing yet I've never seen anyone get into as much trouble as Doc does...

The new faces are probably the only reason the Doctor can keep from getting caught by his enemies all the time. There are so many different versions of him bouncing around space and time, his enemies don't know which one to go after!
