Ianto does have his priorities firmly fixed. I think Jack does know he comes first, but he likes to tease gently and play word games

I think after the way the Doctor has treated him, Jack is unsure whether he deserves Ianto. he needs reassurance that he matters and that he's worth something to Ianto.
