Help is available, dear. In my experience and based on much research, anti-depressants and anti-anxiety medications have come a long way over the years. Since there's a bio-chemical reason for serious depression and anxiety, it makes sense that medication prescribed by a knowledgeable doctor is the answer.

You're being unfair to yourself to anticipate "all kinds of unpleasant side effects" and "major sacrifices." The current state of medicine is better than that. All the symptoms you describe sound like they're manageable with medication, in the best sense. If side effects should occur and you report them, your doctor will simply prescribe a different dosage or a different medication altogether. While it may take a little time to find the right combination of meds for you, the bottom line is that your condition is eminently treatable. You do not need to suffer.

If you have rejected a healthcare provider's recommendation of a pharmaceutical remedy, please reconsider. I can't help thinking that the university health service can come up with a successful plan if you are willing. Keep an open mind. Don't give up.
