Hutch's guilt complex?

Hutch in fanfic is so often written as having a massive guilt complex. Could someone point out to me where in canon people are getting this? I mean, White Knight complex, sure -- that's obvious. But that's not the same thing as guilt: he wants to help people, keeps trying even when any realist will tell you it's not going to work, not going to do any good -- but even here it seems to me his reaction to failure is more complex than simply knee-jerk guilt.

In fact I'm having trouble thinking of instances that match up with these sackcloth-and-ashes meets weltschmerz portrayals in fanfic.

Am I missing something? Where does this intense, irrational everything that goes wrong is my fault guilt of fanish Hutch's come from?
