... *checks with the sound switched off*... mysterious poster is correct. There is a delay - Watson doesn't see Sherlock hit the ground. The "rubbish truck" is an open-topped cage-like contraption with bags of something in it. Potentially enough to save a faller? We didn't see Molly in the crowd, and I hardly think she'd be able to convince anyone else to give a false certification of death. Not unless she had something on them. :/ Plus John grabbed his wrist for a few seconds, but then John's state... and I suppose there could still have been a pulse for a minute, maybe (I know nothing about medicine), so if John felt one it might not have meant anything. So Sherlock lands in the rubbish truck, which is not in John's sightline (he's very determined that John stand in one precise spot, isn't he?), rolls out... somehow does the blood, which he could've set up before I suppose, and is picked up by the people outside Barts, brought inside (I'd assume) and then everything's taken over by Molly.
Also, someone is suggesting that when the cyclist hit John, John got an injection of something - just something that would mess with his perception and make him more easily convinced that it was Sherlock.
