Voter Fraud

From CNN: "During the 2004 presidential election, one voting machine in a Columbus, Ohio, suburb reportedly added nearly 3,900 additional votes to Bush's total. Officials caught the machine's error because only 638 voters cast presidential ballots at that precinct."

As a programmer I can safely say this is no "machine's error". Firstly I can think of no software bug that would screw up that way (computers are pretty damn good at counting). Secondly on government and FDA projects far more effort goes in to validation than development.
