At the end, Rose and the Doctor get separated on two different dimension. I'm pretty sure Rose ended up on her original plane, and the Doctor on a different one? The one where there were blimps over London?

No, Rose and her Mum are in the alternative world with her Dad - who was a bigwig there but dead in our world.

So, after a final farewell, suddenly Donna in a wedding dress turns up in the Tardis, and the episode ended on a big "What?!"

That was the lead in for the 2006 Christmas special, The Runaway Bride' which introduced Donna Noble as a character.

Then, the next episode, he meets Martha and takes her on board. So what the hell happened in-between?! Donna's there one second, and suddenly not!

See above. They missed the Christmas special.

Also, Martha looks exactly like one of the minor characters in the big Rose finale who was killed by a cyberman - it is the same actress isn't it? I'm not going mad am I? So, the Doctor's now on a different plane of existence, yes? I'm so confused! I think I must be missing bits. Help!

Same actress. Martha's physical similarity to Adeola (the woman killed by the Cyberman) was explained as Martha being her cousin.

They were so impressed by Freema's work they wanted her as the new companion.

The Doctor remains on our plain of existence. Rose is elsewhere.
