While some of what I'm asking may be "yeah, of course, duh" kind of things, I just want to make sure that I understand exactly what's happening.
When you edit a past entry, you are in fact seeing the toolbar item and when you click on it you get the menu which shows pics and keywords.
In a new entry window:
1. Have you selected an account? (In this screenshot, where it says aslj_client@LJ.)
2. Does the toolbar item for userpics appear? (Same screenshot as above, where it says [Default].)
3. Does the menu have the list of your userpic keywords and just doesn't show the images?
4. If you put the mouse cursor over the toolbar button for userpics in either new entries or past entries, do you get the full-size preview appearing below the toolbar?
5. Is it something else that I didn't describe or didn't quite get right?

Could you turn on the "Verbose Logging" option in the preferences, run Console.app (in Applications/Utilities), in the "Filter" search box at the top right corner of Console.app, type in "asLJ", then in asLJ open a new entry window and select your account, then copy/paste anything that shows up in the Console window?
Also, what happens if you open a second new entry window and also select your account there? Do the userpics show up there?

If this does make the userpics show up, then there's a problem in the way your tags are being loaded that needs some further debugging.

This dev build (same URL, newer file) logs more information while attempting to load your existing tags into the tag drop-down. My suspicion is that a particular tag is causing the problem, but I can't tell which tag, so I'm hoping this will tell me which tag or tell me that it's not a particular tag.

By the way why do all userpics have different size(I mean - some of them like 100x100px and some ~20x20)?

I think some userpics show up smaller than others in the menu (but still show up full-size in the pop-up preview when you hover the cursor over the toolbar button) because of some sort of resolution information embedded in the image file itself. I haven't yet figured out how to override that and get all the images to be the same size in the menu.
