For anyone who missed it the first time, Washu has developed a medical questionnaire so that the hospital can better treat and keep track of its patients. Despite how you may feel about doctors and hospitals, it really is in your best interests to fill this out fully and truthfully; you may think that you can afford to wait and only tell to us about some of these things should you ever need hospital assistance, but in a place like this you never know when you'll be brought in unconscious or too ill to properly give a history. For anyone worried about sensitive information getting out, I can assure you that only myself and the other doctors will be able to access your records; they will not be open to the general public.
