3. Name of place of origin? I've been many places, Demon World, Human World, Japan, Kingdom of Ercell, and some others

5. What is your age? Unsure, I know I'm over 300 years of age

7. Medical history? Have flue and tetanus shots. Last November a wall collapsed onto me while in my canine form and caused a concussion. I suffered from retrograde amnesia and believed myself to be a dog. It took nearly a month before I regained my identity. Most my memories have returned but the time right before and after the incident are still unclear to me.
8. Have you gotten sick since your arrival here? Once, aside from sickness of chip removal, I had a cold for a small while but recovered quickly.
