I've figured out how to create some legal documents for myself. In case anyone else (in Indiana) is interested in the same:
Last Will and Testament: I purchased a form and instructions from www.PublicLegalForms.com. I don't recall exactly, but I think it was about $10.
Indiana Power of Attorney for Health Care Decisions
Indiana Living Will Declaration: For both of these, I used the following PDF (which I highly recommend): http://www.ecarediary.com/advanceddirectives/Indiana%20Advance%20Directive.pdf
If you're interested in donating your body to science, look at: http://anatomy.iupui.edu/anatomical-education/anatomical-education-faq/
Also, consider setting up the primary and secondary beneficiaries for retirement accounts, insurance policies, and anything else that let's you set a beneficiary, plus getting things like your house, vehicles, and bank accounts into both spouse's names, if married.
