Writing sample Questions

Hello,
What is the proper way of submitting an excerpt for a writing sample? Should I include a cover page with explaining to them that this is an excerpt? Perhaps provide an abstract too?

I would definitely provide that if you're going to provide an excerpt - ten pages cut from the middle of something is pretty meaningless really. I'd probably include a brief description ("This is the methodology chapter from my senior thesis on kangaroo herding") and provide the original abstract you wrote for the piece as well.
