Writing Sample Format

I'm applying  to MFA in creative writing programs as well as MA programs in English lit and none of my schools have guidelines as to how they want the writing samples. Should I default to at least putting a header with my name on it? A cover page? How should I format these?

just call the school.

Honestly I'd have my name on all application materials so nobody loses anything.

I haven't seen any references to cover pages (I'm also applying to creative writing programs and English lit MA programs! What schools are you looking at and what sort of writing are you interested in, if you don't mind my asking?). I have seen requests to put a header with your name on it at the top of each page. But I would second getting in touch with the department to ask directly. (I really don't understand why all schools don't put this information prominently on their website. Do they like getting endless emails about little details like that?)
