I am sure it is permissible, but I would be in favor of mailing the original because it will look much cleaner when it eventually makes its way to the adcom. Don't forget to provide a stamped/addressed envelope for your recommender to mail the letter once it is done.
Faxing things is usually reserved for emergencies in the graduate application process. So, if your LOR had gotten lost and you needed to get a new copy to the school fast, faxing the form to your reccomender would be ok, but you would have to send a hard copy later.
Since you are not in a time crunch yet, send your recommender a large envelope with all your forms, stamped and addressed envelopes for each, and a list of when all of them are due. I've done this when I needed recommendations from people in other states, and it works fairly well.
