I really like your take on the characters that is, in many cases, so different from what one usually finds. The thing between Italy and Germany comes to mind, and also pretty much everything Canada does.
The whole blood thing is a fascinating concept, really like it (though the first time Arthur licked the blood of Alfred's lips I *did* think of vampires, lol).
And thank you SO much for putting in that little bit of hope at the end. I want to believe that as long as Alfred's people still think of their country and of themselves as citizens of the USA, he can gather his strength and fight back. Because that's what a country's strength is made of, in the end, his people. He'll need time to somehow recover from his addiction to Matthew's blood (though as Matthew pointed out, it'll never be fully gone), but he will.
Mind you, I'm not American myself, but I want to be all patriotic for Alfred right now.
Also, one more thing: I am incredibly interested in how exactly things are between Feliciano and Ludwig. Was Ludwig afreid of succumbing to the idea of Empire again, or why/how did he come to drink Italy's blood and let him represent him?
In any case, beautiful story, wouldn't mind a sequel. =)
