There has been created some semblence of medical records in the Infirmary, and they're kept well by Simon, Tris, Shelley, Padme and Mariel.
But not well enough, as there are simple wording or even advanced diagnostic errors that the average person, and, in fact, most incredibly intelligent people, wouldn't note.
That is why River has appointed herself editor, and thusly is editing the log with a green marker.
